POSITION SUMMARY
Under the supervision of the Director of Admissions and Social Work, conducts biopsychosocial assessments for individuals with co-occurring IDD and mental health diagnoses seeking IDD services. Coordinates the admission process for identified individuals in collaboration with the Program Manager, Nurse and Behavior specialist as needed. Provides support to the individual and their support partners with service planning and crisis planning in collaboration with other disciplines and ensures that appropriate supports are in place. Ensures that staff receive all necessary training in preparation for the provision of fully integrated services to the individual. Provides ongoing assessment after changes in status occur and after hospitalizations. Ensures that nursing, behavior, risk and other necessary assessments are completed by qualified clinicians, and that a fully integrated assessment summary is completed in the EHR. Completes discharge summaries.
PRINCIPAL DUTIES AND RESPONSIBILITIES
Pre-Admission Assessment
• Conducts a pre-admission face to face assessment with the individual and
support partners and gathers relevant information from other service providers,
both internal and external.
• Ensures that other necessary specialized assessments are completed and
incorporated into the integrated assessment.
• Completes an integrated assessment in the individual’s EHR to include a
summary of current outcomes and the individual’s preferences for future
outcomes as well as essential needs.
Admissions
• Communicates assessment information to the Director of Admissions and Social
Work and presents assessment information to the Clinical Leadership Team.
• Attends individual’s initial ISP meeting to provide input regarding service
planning.
• Coordinates an admission planning meeting with the individual and/or guardian
and representatives from the receiving company program.
• Coordinates transitional visits as necessary and coordinates training from
individual’s current service provider.
• Ensures that Program Manager has all information necessary to move forward
with admission.
• Ensures that necessary training is available to all staff supporting the
individual.
Ongoing Programmatic Support
• Conducts ongoing assessment in the six months following admission, annually,
and as necessary.
• Conducts assessment after changes in status or hospitalizations to inform
ongoing supports.
• Completes comprehensive discharge summaries in the individual’s EHR upon
discharge and ensures that appropriate services are in place.
• Ensures that ISP goals are implemented and that community integration
activities are occurring per the individual’s choice
Discharge Planning
• Ensures that discharge planning is addressed at the time of admission.
• Ensures that the individual is offered choices via their support
coordinator/case manager.
• Completes a discharge summary in the EHR and ensures that it is distributed
to appropriate parties.
Credible and Records Maintenance
• Ensures that all assessment information is documented and that all relevant
documents are attached in the individuals’ HER.
Minimum Qualifications Required
• Masters of Social Work and three years of clinical experience providing
direct supports to persons with Intellectual and/or Developmental Disabilities
or in a related area.
• At least one year of experience with assessment and service planning.
• Proficient in all office equipment, such as use of telephone system, copier,
printer, scanner, facsimile, postage, and teleconference set-up.
Working Environment
• Social worker for adults with IDD living in residential settings. Will be
required to visit other service provider sites and/or conduct meetings in
offsite locations.
• Must use universal precautions.
